Licensed Practical Nurse Home Health
About the role
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide direct patient care in the home setting under the direction of a Registered Nurse and in collaboration with physicians and other healthcare professionals. You will support patients and caregivers through skilled nursing services, patient education, medication management, and ongoing monitoring, helping patients achieve the highest level of independence and comfort in their own homes.

Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire OR obtained within 60 days of hire with prior approval.
Advanced Care Life Support (ACLS) is required if applicable to unit. Approved IV Certification, as required by facility. Clean driving record, as required by facility.
Ability to connect to the company’s online database twice daily, as required by facility. One (1) year of nursing experience preferred. Previous home health, hospice, long-term care, or community-based nursing experience preferred.
Experience providing patient and caregiver education preferred. Providing skilled nursing care in patients' homes. Monitoring patient conditions and reporting changes to the RN case manager or physician.
Assisting with medication management and patient education. Reinforcing treatment plans and teaching patients and caregivers. Performing wound care and other treatments within LPN scope of practice.

What you'll do
- Providing skilled nursing care in patients' homes.
- Monitoring patient conditions and reporting changes to the RN case manager or physician.
- Assisting with medication management and patient education.
- Reinforcing treatment plans and teaching patients and caregivers.
- Performing wound care and other treatments within LPN scope of practice.
- Documenting visits electronically and coordinating care with the interdisciplinary team.
